Fat below the skin (subcutaneous fat or adipose tissue) is organized into predictable partitions by collagen sheets of fibrous tissue (septa). Fat has a number of functions that include:

  1. Skin and fatty layers prior to VASER Liposuction
  2. Emulsification of the Intermediate and Deep layers of fat after treatment with VASER.
  3. Emulsification of the Superficial layer of fat with VASER and liposuction of the Intermediate and Deep layers. The bulk of the fat in the lower layers has been removed with the liposuction. Note that the connective tissue and nerve tissue remains intact.
  4. Liposuction of the Superficial layer. This also removes bulk, but also allows the skin to contract horizontally, tightening the skin. Again, connective and nerve tissue remain intact.

Fat Section

A large cross section of fat can be sub-divided into fat sections by tangentially and obliquely oriented septae. Blood vessels, lymphatics and sensory nerves are located within the fibrous dividers that partition the fat into fat sections.

Fat Pearl

Fat pearls are a subunit of the fat section. Fat pearls can vary in size in different anatomical areas and seem to increase with varying levels of obesity.

Fat Lobule

Each fat lobule has a separate nerve, small artery and vein. This is a grouping of fat cells that share a single blood supply.

Fat Cell (adipocytes)

The individual fat cell is supported by capillaries that flow adjacent to them.

This slide depicts the results from a VASER Liposuction procedure. There is an overall reduction of fat volume from the Superficial,Intermediate, and Deep layers. There is a contraction of the connective tissue in a vertical as well as a horizontal direction. The skin contracts horizontally.

The neurovascular bundle contains small arteries, veins, and nerves.

C. Septae

The septae are fibrous connective tissue bands that span the fatty layer. The septae connect the dermal layer of the skin to the fascia of the underlying muscle layer. Areas of very thick connective tissue between the skin and underlying fascia form structures such as the breast fold and buttock fold.

D. Fat lobule

The fat lobule is a cluster of fat cells that share a single blood supply.

E. Muscle fascia

The muscle fascia is a layer of dense connective tissue that surrounds the muscle layer.

CI 2 Infusion

Tumescent Fluid Infusion

This is the first step in a liposuction procedure.

The fluid infused into the fatty layer is called tumescent fluid because enough fluid is placed into the fatty layer to make it “swollen”. The fluid is a sterile salt water (saline) from an IV bag that contains very dilute lidocaine which numbs the skin and fatty layer. The fluid also contain very dilute epinephrine (adrenaline) which will make the blood vessels contract which controls bleeding.

The fluid is infused with a “Klein needle” which is a thin blunt cannula. The procedure can be done under local anesthesia fairly comfortably if the infusion is done slowly.

CI 3 Emulsification

Emulsification

This is the second step in a VASER liposuction procedure.

A thin titanium probe is inserted into the fatty layer. The probe is “powered” by an untrasound handpiece that ossilates the probe at 36,000 times per second. One cannot see the probe movement but one can hear the sound.

The emulsification step is important in that it will separate the fat cells and fat lobules from the septae and other connective tissue. Think of shaking grapes off of the vine. This prepares the fat for extraction, making the extraction portion of the procedure easier and more precise. The surgeon is able to remove more fat from a specific area if the fat has been emulsified with ultrasound.

The ultrasound does not disrupt blood vessels like veins or arteries. The nerves are also uninjured. Most of the fat cells themselves are still intact after the emulsification process and can be used for fat transfer to other parts of the patient’s body such as the buttock, face, or breast.

CI 4 Aspiration

Aspiration

This is the final step in a VASER liposuction procedure.

The fat is suctioned out using a blunt cannula. The cannulas used during a VASER liposuction procedure do not have to have sharp edges because the fat is already emulsified. The fat is now easier to suction out and this portion of the procedure can be done more precisely. The surgeon can redirect his/her focus from debulking the fatty area to sculpting the fat, allowing for a better and more refined result.

A significant amount of fat can be removed with aspiration. The fatty layer contains less volume debulking the area. The VASER has stimulated the septae to contract pulling the skin down toward the muscle layer. If fat is removed from the superficial layer of fat, just below the surface of the skin, the skin itself can also contract horizontally more easily.

VASER Hi Definition liposuction requires more attention to detail. It is not simply fat removal. It is fat removal in specific areas, while leaving fat behind in other areas to enhance the patient’s overall physique. Fat may also be transferred from one area of the body to another to add volume complementing liposuction procedure.

During a VASER Hi Definition liposuction procedure, fat is removed from all three layers of fat. A VASER Hi Definition procedure really begins where standard liposuction ends.

The sub-cutaneous layer of fat is divided into three separate layers by connective tissue. The deep layer of fat is usually the thickest. Removing fat from this layer can reduce the most volume from an area of the body. Most liposuction surgeons confine themselves to fat removal from this layer of fat.

Fat removed from the superficial layer is often called “etching”. This allows the surgeon to create more subtle changes in body contour. Shadowing, or the creation of a ”six pack” are examples of the substantial changes that can be created with fat removal from the superficial layer.

Fat removal from the intermediate layer will contribute to volume reduction while also allowing for an enhancement of the underlying natural muscular and bony contours of the body.

  • Shadowing below the external oblique muscle of the abdomen to create definition
  • Creating contrast lateral to the rectus abdominal muscle, enhancing muscle definition. Etching between the muscles as needed to create a “six pack”

Standard liposuction primarily will remove fat from the deep layer of fat. VASER Hi Definition liposuction begins where standard liposuction leaves off. Fat is liposuctioned from all three layers of fat, removing more fat to create shadowing and contouring, while leaving fat in other areas to provide contrast and shape.

Fat transfer is commonly utilized to enhance the final result from VASER Hi Definition liposuction.

1 & 2: Liposuction is performed lateral to the rectus abdominis muscle to create definition and the appearance of youth and athleticism.

Not labeled: The central area above the umbilicus (belly button) is sculpted to enhance the muscle layer as well

3: The sacral triangle is enhanced to create definition between the buttock and the flank region. The sacral dimples can also be brought into relief further enhancing the lower back region.

4: Lateral flank liposuction will move the narrowest point of the waist down toward the buttock. This will lengthen the appearance of the trunk.

5: Medial thigh liposuction and lateral thigh (not depicted) liposuction will enhance the appearance of the buttock. Lateral thigh liposuction will gracefully continue the curvature from the lower back to the lateral thigh, creating a more feminine hourglass shape. This will improve the violin shape that many patients dislike.

Fat transfer to the buttock is often used to further enhance the buttock, giving more fullness and a better shape. This will make the buttock look more athletic as well. Fat transfer to the buttock is often referred to as a “Brazilian Butt Lift”.

The length of the torso (short, medium, and tall) as well as the thickness of the three fatty layers will direct the VASER Hi Definition liposuction surgeon to remove varying amounts of fat and the location and emphasis of fat removal. In virtually every procedure, a goal is to enhance the shape of the buttock.

Once again, fat transfer to the buttock can often benefit the patient’s overall appearance and contour.

Not all VASER Hi Definition liposuction procedures include liposuction of the mons and the medial thigh. Some patients will have more fullness in these areas and not addressing some of the fat here will create an disproportionate look. Removing fat in the mons will help to “rotate” the mons upward, giving a more youthful appearance.

VASER Hi Definition of the abdomen will often be enhanced by liposuctioning just below and lateral to the breasts. This will give more definition to the breasts themselves, and will to a slight degree give the appearance of larger breasts. This diagrams depicts the use of a breast implant which is often considered to enhace the appearance of the breast as well.

An alternative to a breast implant is fat transfer to the breast.

VASER Hi Definition liposuction first enhace the appearace of the breast by giving the breast better definition lateral to and below the breast. Volume can be added to the breast with fat placed below and above the pectoralis muscle and around the glandular tissue of the breast.
